We are located in the Tenth Street Medical Building at 1450 Tenth Street in Santa Monica, near the corner of 10th Street and Broadway. Our suite is on the fourth floor, Suite 404. Parking is available in the building. Our telephone number is (310) 451-8144.

Our office has been set up with your care and comfort in mind. Every effort has been made to take care of your medical needs conveniently and comfortably. Please remember that we are always available to hear any concerns or questions you may have. We appreciate the trust you have placed in us.

Fees
Full payment for all services and laboratory fees is expected at the time of each visit. We accept cash, check, or credit card. At the end of your appointment, we will give you our office visit form that you may submit with a claim form supplied by your insurance company. The insurance company will then reimburse you directly. We will bill insurance companies for surgery fees, office procedures and our hospital visits.

Change of Address and Phone
Please keep us up to date about any changes you have made in your address or phone numbers. We will then be able to inform when you are due for a check-up. If you have changed insurance companies or place of employment, please let us know so that we may update our files.
